When we are looking at treatment for arthritis, it is helpful to distinguish what type of arthritis. Dennis M. Lox, MD, is an expert in Sports and Regenerative Medicine. For instance, in Rheumatoid arthritis (RA) the cytokine profile is different and the effects of elevating certain cytokines such as VEGF may be detrimental. In RA TNF-a and IL-1B are elevated. Enhancing PRP with IL-1 Ra is beneficial as it may be in osteoarthritis.
Various growth factors such as IGF-1 and TGF-B are important in regeneration in OA, but may promote deleterious effects with RA. These factors are important when considering which treatment to provide. Stem cell therapy may be effective if cell numbers are adequate, yet become ineffective due to factors in the environment. Lack of homing can occur due to changes in the niche rendering treatment suboptimal. Understanding this may allow modulation of various cytokine factors to enhance efficacy. Another variable is patients own unique circumstances. Are the inflammatory (Metabolic syndrome), obesity and other co-morbidities, medications, chemotherapy, and radiation affecting treatment or outcome.
